Record what changed — not just what happened.
A useful chronology distinguishes contact/activity from evidence, decision and impact.

What this tool deliberately does not do
It does not infer abuse, diagnose anxiety/EBSA, decide whether absence is justified, recommend legal action, or replace professional safeguarding and attendance procedures.
Where absence may be a safeguarding or welfare signal, follow your setting’s safeguarding procedures and current statutory guidance.
